Background InformationAMDA Mechi Hospital is a 15-bedded charity hospital providing medical services to the general public since 2065 B.S. in Mechinagar Municipality, Jhapa, Eastern Nepal. The hospital is operated by AMDA Nepal with the objective of providing accessible, affordable and quality health services to the community.F. Responsibilities and Duties1. Medical Ward Patient CareProvide comprehensive basic health care to patients admitted to the Medical Ward within the scope of HA qualification, registration and assigned responsibilities.Receive patients admitted to the ward and assist in initial assessment, history taking and documentation as directed by the responsible medical professional.Monitor patients' vital signs, clinical condition and response to treatment at appropriate intervals.Identify changes in patients' condition, deterioration or danger signs and immediately inform the concerned Medical Officer or senior health professional.Assist Medical Officers and other authorized health professionals in examination, diagnosis, treatment and ongoing management of patients.Carry out prescribed treatment and procedures within the scope of professional qualification and hospital protocols.Administer medicines as authorized and prescribed, following appropriate medication safety procedures.Monitor patients for therapeutic response, side effects and adverse reactions and report significant findings promptly.Assist with intravenous therapy, injections, wound care and other procedures within the scope of HA practice and institutional protocols.Provide supportive and basic nursing care to patients as required.Assist patients with personal hygiene, nutrition, mobility and other activities of daily living when required.Ensure timely transfer, referral or escalation of patients requiring higher-level clinical intervention.2. Patient Assessment and MonitoringConduct routine patient assessment and record relevant findings accurately.Monitor temperature, pulse, respiration, blood pressure, oxygen saturation and other relevant parameters.Monitor intake and output, fluid balance, pain and other clinically relevant observations as assigned.Observe patients for signs of infection, respiratory distress, altered consciousness, bleeding, dehydration or other complications.Ensure timely reporting of abnormal findings to the responsible medical professional.Maintain appropriate continuity of care during shift changes and patient transfers.Assist in monitoring high-risk and vulnerable patients according to hospital protocols.3. Treatment and Clinical ProceduresAssist Medical Officers and senior health professionals during clinical procedures.Perform procedures within the authorized scope of HA practice and hospital protocols.Assist in wound dressing, specimen collection, catheter-related care, oxygen therapy and other ward procedures as assigned and authorized.Ensure appropriate preparation of patients, equipment and supplies before procedures.Maintain aseptic technique and infection prevention measures during procedures.Observe and report complications or adverse events associated with procedures.Ensure proper disposal of used instruments, sharps and biomedical waste.4. Emergency and Critical Patient SupportRecognize emergency signs and provide immediate basic emergency care within the scope of training.Respond promptly to deterioration in patients' clinical condition and immediately notify the Medical Officer or senior health professional.Assist in cardiopulmonary resuscitation and other emergency procedures according to training and hospital protocols.Ensure availability and readiness of emergency equipment and essential supplies in the Medical Ward.Assist in stabilizing patients before referral or transfer where required.Participate in hospital emergency, disaster and mass-casualty response activities when assigned.5. Medication and Treatment SafetyAdminister medicines only as prescribed and within the scope of professional authorization.Follow appropriate procedures for verifying the patient, medicine, dose, route and timing before administration.Maintain proper medication records and treatment charts.Monitor patients for adverse drug reactions and promptly report any suspected reaction.Ensure appropriate storage, handling and accountability of medicines in the assigned ward.Report medication errors, near misses, shortages or other medication-related concerns immediately to the appropriate authority.6. Infection Prevention and ControlStrictly follow hospital infection prevention and control policies and standard precautions.Practice proper hand hygiene before and after patient contact and clinical procedures.Use appropriate personal protective equipment according to the level of risk.Maintain aseptic technique during clinical procedures.Ensure proper segregation and disposal of biomedical waste and sharps.Maintain cleanliness and hygiene of the Medical Ward and patient-care environment.Identify and report potential infection risks and unsafe practices.Participate in infection prevention monitoring, training and improvement activities.7. Ward Coordination and SupervisionAssist the Medical Officer/Nursing In-charge in the day-to-day coordination of Medical Ward activities.Support effective allocation and coordination of assigned ward duties among available health personnel.Provide guidance and practical support to junior HAs, ANMs and other support personnel working in the ward.Monitor the timely completion of assigned patient-care activities.Ensure continuity of patient care during shift changes.Participate in ward rounds and communicate relevant patient information to the responsible medical team.Assist in maintaining discipline, teamwork and professional conduct within the ward.Report staffing, operational, clinical or patient-care issues to the appropriate authority.Assist in orienting newly assigned staff to ward procedures and routines.8. Patient Admission, Discharge and ReferralAssist in admission and orientation of patients to the Medical Ward.Explain ward routines, basic patient-care procedures and relevant instructions to patients and attendants.Assist in preparing patients and documentation for discharge.Provide patients and caregivers with appropriate discharge and follow-up instructions as directed by the medical team.Ensure that required medicines, investigations, referrals and follow-up arrangements are communicated appropriately.Assist in referral and transfer of patients to higher-level facilities when required.Ensure appropriate handover of clinical information during patient transfer or referral.9. Health Education and CounselingProvide patients and caregivers with appropriate health education related to their illness and treatment.Counsel patients regarding medication adherence, nutrition, hygiene, lifestyle modification and follow-up care as appropriate.Educate patients and families regarding warning signs and when to seek immediate medical attention.Promote preventive health practices and healthy behaviors.Support health promotion and patient education activities organized by the hospital.10. Documentation and ReportingMaintain accurate, complete and timely patient-care records.Record vital signs, observations, treatment, procedures, medication administration and other relevant information appropriately.Maintain ward registers, treatment records, referral records and other assigned documentation.Ensure proper handover of patient information between shifts.Prepare and submit daily, monthly and periodic reports as assigned.Maintain confidentiality and security of patient records.Assist in collecting ward statistics and service data for hospital reporting and monitoring.Report adverse events, patient safety incidents, complaints and other significant occurrences promptly.11. Ward Supplies, Equipment and InventoryMonitor the availability of essential medicines, medical supplies, consumables and equipment required in the Medical Ward.Maintain appropriate records of ward supplies and equipment.Prepare and communicate timely requests for replenishment of medicines, consumables and other supplies.Check the condition and functionality of ward equipment and report defects or maintenance requirements.Ensure proper use, cleaning, storage and accountability of medical equipment.Assist in periodic physical verification of ward supplies and equipment.Minimize wastage, loss, damage and inappropriate use of hospital resources.12. Quality Improvement and Patient SafetyPromote safe, respectful and patient-centered care in the Medical Ward.Participate in quality improvement, clinical audit, patient safety and monitoring activities.Monitor compliance with ward protocols, standard operating procedures and clinical guidelines.Identify gaps in patient care and recommend practical improvements to the concerned authority.Participate in review of adverse events, near misses and other patient safety incidents.Assist in implementation and monitoring of corrective and preventive actions.Promote a culture of teamwork, accountability and continuous improvement.13. Training and Professional DevelopmentParticipate in in-service training, continuing professional development and clinical learning activities.Support orientation and practical learning of junior health personnel and newly assigned staff.Share relevant clinical knowledge and good practices with ward colleagues.Maintain professional competence and keep updated with relevant clinical guidelines and hospital procedures.14.
